R11 - Customer Advises Entry Not in Accordance with the Terms of the Authorization

What this code means

An authorization genuinely exists, but this particular entry departs from it: wrong amount, wrong date, or some other defect. Since the 2020 rule change R11 was carved out of R10 precisely to separate 'you got the details wrong' from 'I never authorized you'.

Typical causes

• Debited a different amount than the customer authorized
• Debited earlier than the authorized date
• Incomplete transaction, or a debit taken after the source document was improperly reinitiated

What to do

Special case: the Originator MAY correct the underlying error and transmit a corrected entry within 60 days of the R11 return WITHOUT obtaining a new authorization. This is not counted as a reinitiation. R11 nevertheless counts toward the 0.5% unauthorized-return threshold.

What it looks like in an ACH return

The return arrives as an Addenda Record (type 99), 94 characters wide. The return reason code sits in positions 4-6.
