The accountholder (or the beneficiary entitled to the funds) has died. The RDFI will not post further entries to the account. This rejection is final: resubmitting the same payment will fail again — resolve the underlying situation first.

The accountholder (or the beneficiary entitled to the funds) has died. The RDFI will not post further entries to the account.
Typical causes
- Death of the accountholder
- Recurring benefit or pension credit continued after death
- Recurring debit continued against a deceased customer's account
What to do
Do not reinitiate. Stop all future entries and route the matter to the estate. Credits sent after death (e.g. benefit payments) are commonly reclaimed.
What it looks like in an ACH return
The return arrives as an Addenda Record (type 99), 94 characters wide. The return reason code sits in positions 4-6.
